I have checked your account and it seems it was suspended as suspicious of phishing. It seems your form is using the upload field to upload ID's which violates our terms.
The upload field is not allowed to collect sensitive information such as passports, visas, social security card, driver's license, credit/debit cards, bank statements, voided cheques, birth certificates, marriage certificates, death certificates, medicare and photo IDs.
To lift the suspension, we would have to remove the upload field in your form.
Looking forward for your response.

Hi @abishek,
I went ahead deleted the infringing field and reactivated the account. Please try logging in again.
Also, please refer to our Terms of Use to prevent this issue from happening again.
