Why is my account suspended?

Hello,

When i try to show Jotform to my friend.

Jot form Banned me...

Your account is suspended because your "Ürün Sipariş Formu" form is used to collect ID Number which is against our Terms of Use. 

If you want to collect ID Number, you need to add the Terms & Conditions and Signature widgets to the form, for users to confirm/allow/agree that you are collecting the information for a purpose.
