I need help creating complex conditional form

    Good Afternoon,

    I am trying to create a screen print and embroidery pricing tool using jotform. Our current tool is built in excel using various formulas such as VLOOKUP+MATCH, IF, AND, OR, (formula of example attached)


    I need to lookup data from a spreadsheet based on multiple criteria from customer selection such as quantity, number of colors on design, number of ink changes, location of design. Each of these selections reference a unique price that the customer would need to see.

    Is this achievable via Jotform? I'd be happy to discuss over the phone or do a screenshare to walk through our current tool and rule sets.

    Jotform currently has no support over the phone. The best way to reach us is through the support forum. You can also contact us at support@jotform.com, but we do recommend posting your questions through the forum instead.

    It will help us provide you with the appropriate workaround/suggestions if you can elaborate further on what you wanted to do using the form.

    Perhaps, you can create a screencast to explain your requirements and share with us the shareable link so we can watch it on our end?

    Okay no problem, I'll try and explain differently.


    These are the fields the customer enters

    Quantity: 24-10,000 shirts/hoodies etc

    Location of design  (there can be up to 6 locations for design): For certain locations such as sleeve there is a cost of $0.45 in addition to the base cost.

    Number of colors for design : Certain quantities are only allowed to have maximum number of color. For example 24 quantity max number of colors is 4 while 144 qty the max number of colors is 8. Each color is a different price point.

    So for example. Customer wants 24 shirts 1 color design 1 location. It should output cost of say $5. and say the customer decides they want that 1 color 1 location design to be on the sleeve, it would then adjust to output $5.45 cost if they selected sleeve print

    the tool also needs to be able to output a price for a customer who wants 500 shirts. The shirts are 3 locations. 1st location is a 1 color sleeve, 2nd location is 4 color back, and 3rd location is 2 color front chest. This would take these 3 variables and lookup the price from an excel table and output cost of say $3.27

    Any ideas?

    Few questions regarding your requirements:

    For certain locations such as sleeve, there is a cost of $0.45 in addition to the base cost.

    - Is your customer the one will provide the location and quantities?

    - Also, do you have a website or layout that we can look at so we can visualize the form you want?

    - Is this the form (92394849700163) you're working on? 

    By the way, I think your requirements will be met using conditional logic because that's what you'll use to check the quantities and location to assign a max number of colors.

    -yes, this will be a drop down or check box indicating that they want us to print the design on the shirt sleeve, front, back , etc

    - Also, do you have a website or layout that we can look at so we can visualize the form you want?

    -I am not looking to replicate the layout we currently have, it is more important the the features and pricing are functional. I've attached a screenshot below of what our current excel tool looks like

    yes, this is the form I started to play around with today.


    The difficult part for me is the multiple variables in designs. I have entered the various prices based on color and quantities as a hidden spreadsheet on the form.

    Sorry for the delay. I'm inspecting your test form: https://www.jotform.com/92394849700163 and there are many location labels so I don't know which one you are referring to in your explanation before.

    Can you please elaborate on your requirements or give us an example using your test form? You can screencast or screenshot the selection you made and tell us the output you want.
