Updated an email. address for a user account and lost access to forms

    lost the ability to access submissions as well

    I do not see any forms in your account (nmhgqagreenville). If you mean other account, please provide the username or the e-mail address of the account.

    If you can not login to your account, please try to use the Forgot Password button and reset your password.
