I have a free account and five forms, four forms created. I am not receiving submissions from one form, the others have been set disable. I can't find a mistake, have tried several different notification email addresses, checked that they are not on the bounce list, they are not going into spam or junk folders. Help please?
My form name - SEA Site Visit Notification 2019

When I checked your e-mail logs, I noticed that you have received notification emails from other forms as shown below:
Also, I have checked your "SEA Site Visit Notification 2019" form. It seems that you have not created a notification email for this form.
You only created two autoresponder emails as shown below:
