Account suspended

    Hello,


    My account is showing it is suspended and I can not log in. I reset my password and still is showing as suspended. Can you please let me know how I can regain access?

    Your account is suspended as it was suspected to contain Fake Service form - West Bloomfield 2019 Guys Summer Trip form. 

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms of Use

    In order to reactivate your account, we will have to delete the offending form.

    Or, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    Send these proofs to reviewers@jotform.com with Thread #1945048 as your subject. We will let you know about the status of your account after a thorough review of your proofs. 

    You will also need to change the email address of your account to a corporate one after your account is reactivated.
