Respondents are intermittently NOT receiving a certificate

    I run educational programs for healthcare providers.  After the education, they fill out a series of questions and submit.  They then are sent a Continuing Education (CE) Certificate.  Approximately 10% of the people responding do not receive a certificate.  

     

    Please help.

    I have tested your form and I was able to receive an autoresponse email from NetworkCE@avainfo.org.

    Do you have a list of emails or sample emails that were having an issue receiving autoresponse emails using form? The issue could be related to their domain. Please share their emails so we can see if there's a pattern like one domain is blocking emails coming from your domain.
