Unable to login. Account was suspended.

    Hi, when I try to log in I get a "login failed - suspended" error. Is there an issue with our account, or with the site? How can I get re-activated again?

    All of your forms were flagged as a phishing form by our reviewers team. They found that your forms were collecting bank information which is against our terms of use.

    As per our terms of use https://www.jotform.com/help/8-Terms-of-Use:
