    eu uso os formulários 

    pra cadastra cambista 

    só peço login e senha pra poder ser criado um acesso pra minha plataforma

    The reason your account is auto-suspended is due to our phishing detector noticing your forms asking for login credentials as well as Bank Receipts



    This goes against our Terms of Use, Before we can re-activate your account please agree on our Terms of Use and we will delete the guilty fields.

    Sorry, but as per our Terms, we do not allow our customers to collect login credentials via forms.

    Phishing. You agree that Jotform, Inc. may terminate your Jotform Service immediately if a form's purpose is found to be deceptively obtaining, for example, sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the Jotform Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials.

    As you can see above, the bank receipts again fall under "other sensitive personal information".
