Hiding entire page based on another field's answer

    I am creating an expense report. Those completing the forms can choose 1, 2, or 3 types of expenses to report on one report. 

    1. Credit Card
    2. Check Request
    3. Milage

    Credit Card - needs everything but should skip all of check request and milage
    Check Request - does not need mileage needs everything else.
    Milage - needs everything except check request information

    I have tried to set this up but as you go through the form towards the end you see Back and Next with no page. If I don't put this here the form stops on this blank form? How can I fix this.

    Also

    How can I fix the first date on page 1?
    I have shrunk the question but i need to see the full date.

    Instead of using the SHOW/HIDE A PAGE conditional logic, please use the SKIP TO/HIDE A PAGE conditional logic.
