To my understanding, you would like to show different fields questions in your form based on the job your respondents will select. That would be possible with the help of conditional logic.

As for placing your form to your website, yes, it is possible to embed your form to your website and I would suggest that you check the following guides.

I would like to apologize for any confusion.

Are you looking for a way to find which job the submitted form is for?

May we know how the jobs are separated on your website? Do you have different pages for each job and on each page, the same form is embedded?

Also, how do your respondents get to the page where the form for a specific job is located? Do they click a link from the main page of your website to get to where the form is embedded?

If, in any case, I have misunderstood your concern, please feel free to let us know and give us more details so we can help you further.

    Thank you for the clarification.

You can add the fields to your form on its builder page.

Once you created a form, you will be automatically redirected to its builder page.

If you have an existing form, you will need to click the form on the MyForms page (https://www.jotform.com/myforms) to select it and then click the "Edit Form" option after.

Then, on the builder page, click the "Add Form Elements" button on the left to add fields to your form.

Your questions are a bit vague. Would you mind elaborating it further? If you are using your own custom form and you want the submissions to be submitted in your JotForm form, it would require programming knowledge to integrate that. You can use either javascript or PHP script to submit the data from your custom form to your JotForm form through API. Here's the endpoint https://api.jotform.com/docs/#post-form-id-submissions
