Terms and Conditions widget not appearing on live website

Hello,

I have created a form and have integrated the terms and conditions checkbox widget at the bottom of the form.

The widget appears in the jot form editor, but not on the live implementation. 

Could someone please respond to troubleshoot this issue.

Note: This website is still under construction. Please do not submit an entree. 

I checked your form and it seems that you already removed the widget. Can you please add it back so that we can investigate the issue?
