Hide & show question when answered yes or no.

How do we fix this situation? If I answer a question a yes then subquestion B show and I answer yes that show upload option. Now I realized I needed to answer question A no instead of yes. But in this situation, the upload option still shows..

I would recommend you to try clearing the hidden fields, this way the selection on these fields is cleared and the fields are hidden again. 

To clear hidden fields, you may go to the settings tab, click on the form settings section and scroll down until the "Clear Hidden Fields Values" where you should select the "Clear on being hidden" option:

Will this work on "Save and continue" form??

Clear hidden fields basically will clear the values when a field as soon as it gets hidden. So even before submission, it's already empty. This can be used when there are other conditions that require the field to have different content. So with this option. The time that it becomes empty is upon the action of being hidden.

So the save and continue form should still work. It would be after the user went back to change the answer for the other question.
