    Thank you for your response.
The form started formatting correctly yesterday, but today it is back to
    the formatting issue.
We have also had a host of other issues including that some of our
    submissions have not been delivered.
Can you confirm that you are doing server work on your end? Also can
you please let me know when we can expect to have service return to normal.
    Thanks!

The form works fine when I try it standalone:

    http://www.jotformpro.com/form/30097082269962

It seems to be broken on your site. You are using iframe code so the height is hardcoded. And the size of the form seem to have increased probably because you modified the form recently. So, you will need to increase the iframe height size. Edit the form code on your site and increase the iframe size to 675 or 700 just to be safe.

You also seem to have some kind of static height on the page. You must also increase that size so that the form can fit there.

On regards to your first 2 questions:

    1) Forms submissions not being delivered

Can you please set the Sender email address to  noreply@jotform.com ? Otherwise submisions might be rejected by your email provider servers, making the submissiosn to fall into the spam folder or even more to fall into our bounce list.

Please follow a guide we have prepared to prevent email issues

    2) Unable to get submission infomation through your web interface

You can find your submissions following these steps

    1. Click My Forms on top of the page

    2. Click the form of which you need to get the submissions.

    3. Click the Submissions button from the toolbar above

If you still need assistance, let us know
