We would like to apologize for any inconvenience. I have tried connecting my own personal Square account with a Sample Form and I didn't encounter any issues doing so.
I would suggest that when connecting your Square account, please try switching the mode first of the integration to "Test" then switch it back again to "Live" and log in your account.
