Confirmation emails are not being sent!
Test confirmation emails work.
But when I add a submission myself, I never receive a confirmation.
Can you please advise?

Thanks for posting. Unfortunately I can receive your auto-responders on both Dallas and Houston forms. See image below
Your autoresponder is working fine but your notification, I noticed it has a blank Recipient Email at the bottom right corner. This probably the cause of you not having any notifications everytime someone fills up the forms. Please put your email there or any email address that you want to receive notifications into.
