Can I use Jotform for employee onboarding (collecting confidential information)?

    I am considering using Jotform to collect employee onboarding paperwork. Some of the confidential information that we would need to collect include:

    - W4 (including Social Security number)

    - Direct Deposit Authorization (including bank account and routing numbers)

    - Health History Forms

    Are these items acceptable to collect via Jotform, assuming we have the HIPPA-compliant and encrypted employee information form?

    Yes you may be allowed to collect such information if your form complies with our security policy.

    For Health data, please upgrade to HIPAA compliant package (Silver and up), then enable HIPAA compliance forms.

    For other data, please add the Signature and T&C widgets. For added security, we also highly suggest encrypting your form.

    Please note that although we may allow asking for sensitive information, however, we highly discourage any digital upload of sensitive information (eg: driver's license, etc).
