How do I add a processing charge to the Form?

Good Day, 

I'm attempting to use the Form Calculation Widget to add up a price total - the 4.5% processing charge and need assistance with the equation.

The Number should be plugged in to the Total Field under CashApp Payment Option.

3. Calculate Tax on the total.

4. Pass the total value to the payment field.
