Suspended Account

    Hi, I am a happy new jotform customer but when I tried to log into my account tonight it says it has been suspended. Did I do something wrong? How can I get my account back?

    We have checked your account and see that it is suspended due to the form https://www.jotform.com/build/21571004212335 which is collecting credit card information.

    Please note that it is not allowed to collect sensitive information such as credit card information, user login forms, etc using JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please check our terms of usage for more information on phishing forms. However, you may be able to collect SSN for certain forms and business type. Please see the following section provided below from Terms of usage.

    You can eliminate these information fields, or you may let us do it for you. Then we will be able to activate your account.
