Why my account is suspended?

    I just signed up for Jotform, but I am getting an error that says Suspended when i try to sign in.  Please advise.

    Your account was suspended because your form 92458796866178 was considered as phishing one as collecting Tax Identification Number is a violation of our Terms of Use:

    Phishing. You agree that Jotform, Inc. may terminate your Jotform Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the Jotform Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of Jotform, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    If your purpose is not to collect this data from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    In other cases, our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Or we can just delete the field and activate your account.

    Ok. The terms and conditions widget is on the form as is the docusign widget and it is connected to our corporate account. I will update the email address to my corporate address.

    I deleted the guilty field and re-activated your account. If you wish to add the field again, please consider adding the required widgets and changing the account email address.
