creating review/audit with scores form

I need to create a form to review staff but need SCORES as results. Plus, i need to be able to compare previous scores with current scores. How do you suggest I do this?

So to clarify, you would want to have a form with fields that have assigned value and to have your form calculate a final score.

One option is to set up your form like a quiz or test and you can use the: Turn-Your-Form-into-a-Quiz-or-Exam- app to do this. 

The other option is to set up your form to perform Calculations using the Form Calculation Widget.

So each question would be set with a different value and would calculate depending on the user's answers.

As afar as comparing scores, you are able to run reports on your submission data that might meet your requirements.
