Why is our account suspended?

    Hi!

    Our account is suspended. We believe we follow the terms of use. Please help us because there are important forms in our account.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting sensitive information is considered as phishing activity, as stated in our Terms of Use.


    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.


    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things.


    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.


    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.


    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.


    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.




    Please confirm that you can add these widgets to the form or delete the guilty fields along with changing the account email address so that we can activate your account.

    Thanks for your respons. We would add these widgets and change our
    mailadress to a company mailadress. How we can change it because we're
    suspended?
