Form timing out after submit

We have an application form on our website, and a couple of applicants have emailed saying when they submit the form, it just keeps getting stuck on the "Please Wait" screen after form submission. It has sometimes stayed like that for 20 minutes. 

I've attached a screenshot of the console errors that appeared on that page. 

Can you please advise? 

I did not encounter an error when submitting the embedded form.

If some users still encounter the same problem, you may consider using iframe embed code. If the error was caused by a script conflict, this will not be an issue with iframe.
