Pre-populate form fields and complete by another person

For example I have a patient that is having surgery and I want to pre-populate the form and send it to the to finish filling

There are two ways to achieve this requirement. 

First is to send that person a pre-populated URL. Accessing the URL will show him a form with some fields pre-populated. Please refer to this guide about Prepopulating-Fields-to-Your-JotForm-via-URL-Parameters.

You can also use the process workflow method. Please refer to this article on how's it done: General-Approach-on-Building-an-Approval-Process-Workflow.
