Form is disabled and account autosuspended

    Hello Polat,

    It seems the said form (ID: 92526393918973) in your account was flagged by our phishing detectors and your account was autosuspended for suspiciously collecting bank information from respondents. This is a violation of our Terms of Use as shown in the highlighted section below: 

    Phishing. You agree that Jotform, Inc. may terminate your Jotform Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately. 

    This is probably seen in the fields of the form as shown below:

    For us to reactivate the form and reinstate your account, we will need your permission to delete these offending fields so that your forms will not violate our terms of use. If you have any need to collect payments from your respondents, you can try using our PCI-compliant ways of collecting payment by integrating our payment apps into your form. You still won't be able to collect bank information directly though. 

    We await your response and permission to remove the offending fields in order to have your account and form reinstated.

    In fact, the only thing I wanted to know was their IBAN number. But I'll
    still fix it, Will my form be automatically available when I fix this
    problem? Also, I'm sorry I can't log in to my Jotform account for
    blocking. Will you grant me access?

    I removed the guilty fields.

    Your account has been restored. Please avoid collecting such information via our forms or your account can be disabled.
