Numerous failed submissions on mobile devices. Please let us know how we can fix this ASAP.

When I test a cloned version of your form it seems to be working properly on my end.

Are you still able to reproduce the submission issue on your end? What device and browser do you use? Are you accessing the form using a browser, or through some app for example?
