Suspended Account

HI, I was actually hoping to login and delete an account I no longer need, and when I tried to login it said that login failed because the account was suspended. Can you explain this to me? 

I have checked your account Missourimidwives and it seems to your form https://www.jotform.us/form/90367165798169 is suspicious of phishing as it seems to be collecting banking information which violates our terms of use.

In order for your account's suspension to be lifted, we would need to remove the banking fields in your form.
