Hello,
Results are not being saved for a checkbox with a single option.
Repro.
(1) Check single option for Documentation?
(2) Check one or more options for Compliance Checks Completed
(3A) Go to submissions and check the relevant columns. Nothing is stored in Documentation?, but options checked are stored in Compliance Checks Completed.
(3B) Load the edit link. Documentation? option is not checked, but Compliance Check Completed options are checked.
Note. Checkboxes by Complete/Initials are widgets rather than standard checkboxes. The widgets seem to work fine as single options.
Config. Windows X and (current) Chrome browser. Also tested on older Firefox.

Hello,
I do not seem to understand your query. Please, tell us exactly what you wish to achieve with your form and how you would want it to behave ideally. This is to enable us to get a picture of what you really want to get out of the form.

Sorry for the confusion.
We have a form which has a checkbox with a single option. When we check that option and submit, the value is not stored in the JotForm submission.
Other checkboxes (with multiple options) are stored on submit.
Please let me know if you cannot repro these results following the steps in my first post!

Are you referring to this Single Choice field?
Or this Multiple Choice field?

Sorry for the confusion!
The single choice checkbox that has the problem is Documentation? (see screenshot). This box can be checked, but does not get saved when the form is submitted. As such, when user use the edit link, the box appears unchecked.
As I mentioned in above post, the single choice checkboxes under Complete/Initials currently work fine, but they are a widget (not the default type).
Obviously, I could switch Documentation? to that same widget to fix my problem, but I'm trying to help JotForm fix a problem.

Hello,
I have reviewed your form and I can note that the "Documetation?" field does not have options for the email to report. Other checkboxes have options that can be reported as fields on the email. So, you could consider adding the "Y/N" options like other similar fields.
Please, try this and let us know how it works for you.

I added an option label (Y) and that works =)
Also, since JotForm does not store the user response without an option label, I would recommend the devs modify this component to prevent users from having a checkbox without an option label.
