We need more details to understand the problem better. We would appreciate if you could answer these questions;
1. Did your patient click the submit button after signing it? Can you elaborate more about what exactly happened after signing the form?
2. Is the form filled using the JotForm mobile app or just a mobile browser?
3. If the form is also embedded, please let us know the website URL.

He was using a mobile browser on his iPhone.
We submit the form to the pt via an email, so not embedded in this application.

Can you share some more information on the submission we are looking for? Have you or your patient received a confirmation email for it?
