Can the form be completed twice?

Good evening,

I have created my form and sent to parents of my netball club.  I have some parents who need to fill the form out twice as they have two children at the club, however the form won't let them do this.  Please can you advise me on how to let them complete two forms?

I see that the form is enabled with 'Check cookies only' option in the Unique submission setting.

You may disable it from the form settings so that the form can be submitted multiple times.
