form troubleshooting

I was using this form builder in connection with wix and for whatever reason the form has dissapeared on the site. I dont remember quite how i set this up but i know that i copied the html code from jotform. what do i do?

There are a few different options to embed your form onto your website. In Form Builder, click on Publish and the Embed to view the code options.
