I am not able to replicate the issue. The form is displaying fine without any problem.
Could you tell which browser you are using? Also, please check with a different browser.

to convert to the form and saw its beta.

I also tested your form in preview mode and it seems to display correctly. I could be a temporary issue.
Do get back to us if the issue persists and we will take a look.
