My form is not working it is showing up blank!

I am not able to replicate the issue. The form is displaying fine without any problem.

Could you tell which browser you are using? Also, please check with a different browser.

I just re- created it. I’m assuming it didn’t work because I uploaded a pdf
to convert to the form and saw its beta.

I also tested your form in preview mode and it seems to display correctly. I could be a temporary issue.
