What is the fillable forms pricing?

I only want to use the fillable forms.  I do not need payments etc.  Is there a monthly pricing plan for this basic usage?  Or there is no choice but to sign up for the $15.83 plan?  

Also, if I pay for the cheapest monthly plan, does the JOT FORM branding get removed from all forms and outgoing emails?  I don’t want the branding on the auto emails sent to customers as it is embarrassing to see ‘free jot form’ logos, etc.

If you upgrade to any plan, you can remove the branding in your forms and emails.
