StevenAsked on September 16, 2019 at 1:07 AM2. When I receive an E-mail that the form was filled out.. I see things on the form from the previous form I edited. How do I delete these things (They are items that reference office supplies - from the template I used) this shows up in the E-mail that I receive. I will show you a screen shot. When I look in the inbox all the information is there. Pleas see screen shot.

I am sorry for the trouble caused to you. It seems you have edited form form's notification email alert template and that is the reason why changes made to your form is not being displayed in email alert.
To solve this problem, please delete your form's email alerts and add new one. When you add a new email alert, it will include all the questions of your form. The following guide should help you:
