How can I add SSN field?

Hi there,

I'm trying to create a application form for employment in my organization. this form require some sensitive data such as SSN & Alien number. How can I make that field secure or if i'm allow to request it.

I have upload it the form in PDF.

Collecting SSNs through our forms is considered as sensitive information as per our terms: https://www.jotform.com/help/8-Terms-of-Use

If your purpose is not to collect SSN from the general public, but from your own customers and you comply with recommendations below, then we can make the exception:

1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

2. Add a Signature widget where your users sign and agree to provide such sensitive information.

3. If possible, change the email address on your profile to one that shows your business domain rather than the public email address you currently show.
