Conditional Form - Drop Down with different criteria for each selection

    I would like to list an award category selection, where when each was selected it's criteria information is populated. This allows the user to see the information before making the selection.

    How can I do this?

    Currently, I use a separate form for each award, but want to combine so that there is just one form collecting all of the data.

    Listing our URL to our page - with all of the awards listed and a separate jotform link is also listed.

    Would like to function like the screenshot attached. This would allow us to have one form and one link.

    You may add different Text elements to your form, and display them based on conditional logic.
