Script Embed : Form not displaying properly on a webpage

    Hello!

    I have copied and pasted the below script into the correct area on my site, however when it publishes, it is missing vital areas of the form.

    You can see its missing most of the questions

    I looked at your web-page and the form appears to be displaying correctly. I did not see any part of the form is missing there -

    If you still think this is an issue, I would request to embed your form using iFrame embed code. Here is how to get the iFrame embed code - https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

    Could you let us know what part of the form is missing from your website?

    I compared the embedded form with the standalone form and it seems that all the fields are appearing correctly on the website.

    Could you clear your browser cache and then reload the webpage again OR please try to load the page via an incognito window of your browser.

    Hi Heather!

    Thank you for attaching the screenshots. The form that is embedded on your website is the Estimate Your Coverage form. Upon checking the form's Settings>Conditions, I can see that all of the conditions are missing one or more fields, which renders the conditions useless and creates a conflict which is what causing most of the form to not be displaying in Preview mode and on your website.

    If you feel like you might want to use these conditions later, you can simply disable all of them by pointing the cursor at the condition and clicking on the Gear icon>Disable. You can also delete the conditions by clicking on the Gear icon>Delete. After that your form will be displayed as expected. The embedded form should update automatically, but just in case there is any issue, please clear the form caches by following this guide: How-to-Clear-Your-Form-Cache and the form will be updated.
