How to create a file upload form?

Can the person completing my form attach a document to it (say a WORD or EXCEL doc)?  If so, how do I set that up in the form and where do I get the doc in submissions?  Thanks, Caryn

It's very easy to create a form with File Upload. You can use the File Upload field in the form builder to create the form.

Once the form is submitted with the file upload, it can be viewed in the submission sheet in the form of URL. The URL specifies the location of the upload file on the JotForm server. You can easily download it form the submission sheet. Additionally, it can also be included in the email notifications as an attachment.
