Password Protected Forms - Impressions Quota

Does presenting the password prompt for a form count as a form-view that gets deducted from your monthly quota? 

Does successfully proceeding to the form count as a new view, or the same view as the initial password prompt.

If someone tries to enter a password unsuccessfully multiple times, does each refresh of the password entry window count as a separate view against one's quota?

The answer to this should be clearly presented in your materials and the UI.

No, only when the user puts in the correct password and proceeds to view the form would this deduct from your monthly quota.
