Is it possible to create a form that a user cannot submit more than once?
I don't want a person to create false positive or false negative by resubmitting a survey form multiple times.
Also
Is it possible to insure the applicant that I do not require or see their id information i.e. I don't want their email or anything that would personally identify them
Lastly how would or could I have BOTH of those conditions on a survey?

You can consider looking through our guide on How-to-Set-Unique-Submissions-on-a-Form. This feature offers a few options to help you prevent respondents from making multiple submission. I believe this is perfect for your survey.
