Some of the newly added fields are not shown in the Fillable PDF?

I uploaded a fillable pdf form that has several checkbox sections, one after another. When jotform made the new form it combined all the checkboxes under one question. I made separate questions with the correct check options. I also had to edit a couple other fields to the appropriate format. When I preview the pdf the new fields do not associate with the correct questions on my pdf. Is there a way to associate my new fields with the correct existing question on my pdf?

Yes, that is the form.

After the question "During travel to the veterinary hospital, does your pet do any of the following:" it automatically included the checkboxes for the following 2 questions as well. So I created the fields for "Does your pet prefer:" and "Check any situations listed below that your pet has shown avoidance or dislike of in the past. You can add additional comments at the end." but their answers do not associate with those questions on our pdf. I also removed the short answer box for "Anything else you would like us to know?" and replaced it with a long answer box and that does not associate with the question on our pdf either.

First, I cleared your form cache on our server.

After that, I checked the form and all the fields look fine. 

So I created the fields for "Does your pet prefer:" and "Check any situations listed below that your pet has shown avoidance or dislike of in the past. You can add additional comments at the end." but their answers do not associate with those questions on our pdf.

The field is showing properly. 

I also removed the short answer box for "Anything else you would like us to know?" and replaced it with a long answer box and that does not associate with the question on our pdf either.

The field is already Long Text field.

The form looks correct because I edited it and created those fields. Orignally, JotForm created the form with the question "During travel to the veterinary hospital, does your pet do any of the following:" and it included the correct answer choices AND the answer choices for the following 2 sections. Then it asked the next 2 questions, but with no response options. 

I removed the extra answers from that question and added the answer choice fields for the next 2 questions. But now, when the questions are answered, they do not associate with the question on my original pdf.

You can see where on the original form where it wants all 3 questions to associate with only the one question on the new form.

And where the new form answers should be associating on the original pdf.

This field is already Long Text because I removed the Short text Text and replaced it with the Long Text. But now it does not associate with the question on the original pdf.

I just need to know if there is a way to force the answer fields that I created to associate with the correct questions on my original pdf.

I noticed a feature request ticket is already forwarded to the development team to check for the mapping of the Form fields to the Fillable PDF. We will update you on this ticket once we have news on this.
