My form was disabled

    I tried logging in and wouldn't allow me and so I reset my password and I still cant log in. Also the link I sent to my client now says form disabled? Please help!

    Hello,

    You have been suspended due to your forms that are created to collect social security number, driver license number, username, and passwords. Please note that such forms are against our Terms of Use (https://www.jotform.com/help/8-Terms-of-Use).

    We can make an exemption for the social security number and driver license fields, but you have to still delete the file uploads of these IDs, you can only ask the number. However, you have to meet the following:

    1. You must have the Terms and Condition in the form that would state why you are asking this information. A general term for your business will do us.

    2. You have to add the Signature widget as well for people to sign:

    3. You have to use your business email in the account, and not just a free email address. We will use the business email to verify. If you are only affiliated with a business, a confirmation from them is required.

    I can delete the upload document portion of my forms and just request for numbers or will you be
    able to do that for me since I cant access my account.
    As far as terms and agreements, should I email that so you can input it or will you give me
    access so I can do it myself.
    Our business email is info@stocktonhills.com.
    Please advise.

    The DL was needed to properly identify the applicant. And part of our services include applying new corporations for our clients by which our lawyer needs a social security number in order to file an EIN number for that new corporation.

    Thank you. I have just reactivated the account. Kindly check again and let us know if you still cannot log in.

    You will have to add the terms in the form using either the text field or the terms widget. I forgot to mention that you need to remove the Freecreditscore.com fields as well. We do not allow external credentials to be placed in the form. You can ask that through email or phone.

    Upon checking your account, I could find three forms in your account.

    I believe these are the forms that you're referring to. Are you not able to view them on your account? I believe you may have logged out from the Jotform session.

    Please log in again into your account to access the forms.

    Please add the required widgets as explained by my colleague above into the form, so that the account does not get suspended again.

    Hello,
    I was able to access my forms and I deleted the option to upload. I wanted to know if its possible to put a payment in your form so my client can pay directly on the jot form? Is this allowed? I just want verify first.
