How do I make sure that new title and graphic are displaying in the cloned Form?

WHEN I CLONE MY TEMPLATE THAT SAME TITLE AND COVER GRAPHIC COME UP WHEN SHARED-HOW DO I MAKE SURE MY NEW TITLE AND GRAPHIC ARE DISPLAYING? THANKS!
