We would like to apologize for any inconvenience.

I checked the PDF Editor page of the form in your account that I assume you are referring to and found a huge space on its second page. It appears that there is an extra Page Break on the page that results in the huge space. I suggest that you delete the Page Break and before downloading the PDF Form.

I hope that helps solve the issue you are having. Otherwise, please feel free to let us know.

So my editor view did not look like the one you posted. In mine, the date field was all on one line, but then downloaded as in your image. I ended up making the signature and name fields narrower and the date field wider, and that took care of the problem.

Not sure why my editor page showed up differently???

I also tried opening your form in the PDF Editor and was able to see that the date fields are all in one line:

It also looks as is when I download and open it in a PDF Reader.

Do you still encounter the same issue?

No it is now fixed, as I mentioned in my last reply on the thread. I had to make the name and signature fields smaller and the date field wider, then it both looked and downloaded the same. Thanks to the image that was posted by Mike_G, I was able to figure out the problem-although why it did not look the same on my computer as it did in that image, I do not know. The double line date made the page break move to the next page and then another page was added. In my view before I changed the widths, the page break was on the previous page near the bottom of the page and under the last date field which was all on one line.
    Thanks for asking.
