Account or Form Suspended

    Hi Jotform team! My account "FRC2" will not allow me to login. It notes that it is SUSPENDED and I was hoping to reach out and resolve any issue or problems to be able to login to my account.

    Thank you! My other account 'FRC5' is also suspended. Is this for the same reason?

    As checked, your account "FRC5" has been suspended as this form: Event Intake Form contains a Copyrighted Logo. It is considered as a scam and violates our Terms of Use.


    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. 

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    Or, we can just remove the logos and lift the suspension of your account.
