How do I add my form to my website?

This is my first time using Jot Form. I "think" I have successfully

created my form (which is a very simple one) with a means to

collect donations. Is there anyone available who can double 

check this for me?

Please click on the payment field, then on the wand icon, and put your PayPal account:
