Alton Kelley no longer works for our company. He set up an account with you and I need to terminate his access and I need to gain access. Please let me know what needs to be done to make this happen.

Are you referring to the account morrisbusiness? You can reset the password. You can ask your IT team to gain access to his email ID and then you can receive the reset email there. We cannot do much on this.

Also, you can create a new account and clone the Form in your new account.
