My form disabled for no reason

I created a form for our school which is grandeoakbridge Montessori. Now this form is to make it easy for parents to put their data and also create the username and password they will use to view and download their children's school report result. 

After creating the form, it got disabled. I can't start creating username and password for over 150 parents. This will be easy if the parents create their login, so I simply copy and paste when I am creating them as users(Parents) in our school portal
