Notification not received at email inbox

    I followed directions on definig notification email address exactly.  Form appears ast should on Web page, but filling ity out and submitting it reesults in no notification receipt at email inbox.  It shoulg have been sent to

    I checked your form and found out that on the notification the Sender Name is set to empty. It should be set to Jotform, I already edited it for you. I also made a test submission on your form, you should be receiving notification please check your mail.Thanks.
