Attaching a PDF form to a form

Is there any way to attach a PDF form to our form we use for registration?  We are wanting to have the parents fill out a Medical Release form when they register so we can print both the registration form and medical release form out at once.

You can only display a PDF to your online form using PDF Embedder widget. However, users will not able to fill it out. So I think what you can do is to just incorporate your PDF's question into the form, like creating fields that correspond to the questions found inside the said PDF form to your existing web form.

And then attach a PDF copy of the submission to the email notification that you'll receive, which you can download and print. Please refer to this guide on How-To-Include-Submissions-As-PDF-Attachment-In-The-Email-Notifications.

Another method: If the PDF Form you mentioned is fillable, you can convert it to an online web form. Please see this guide on How-Can-I-Import-My-Own-PDF-to-JotForm.
